On July 26, 2005, Space Shuttle Discovery lifted off from Kennedy Space Center on mission STS-114, NASA’s “Return to Flight” — the first crewed launch of the Space Shuttle since the Columbia disaster of February 1, 2003. The 13-day mission to the International Space Station validated a wide range of safety changes made after Columbia, but the recurrence of foam debris shedding from the external tank during ascent prompted NASA to ground the shuttle fleet a second time, exposing the limits of the redesign.
Background: the Columbia disaster
STS-114 had originally been planned as a routine station-assembly flight, but it was reconceived after Columbia broke apart during re-entry on February 1, 2003, killing all seven astronauts aboard. The Columbia Accident Investigation Board traced the loss to a piece of insulating foam that had broken from the external tank during launch and struck the orbiter’s left wing, breaching the reinforced carbon-carbon thermal protection. In the two and a half years that followed, NASA redesigned the external tank to shed less foam, added cameras and sensors to inspect the orbiter in flight, and developed on-orbit tile-repair techniques. STS-114 launched 907 days after the accident, following a July 13, 2005 attempt that was scrubbed by a faulty fuel-level sensor.
Launch and crew
Discovery launched at 10:39 a.m. EDT (14:39 UTC) on July 26, 2005, from Launch Pad 39B at Kennedy Space Center. The seven-member crew was commanded by Eileen Collins, the first woman to command a shuttle mission, with James M. Kelly as pilot. The mission specialists were Soichi Noguchi of the Japan Aerospace Exploration Agency (JAXA), Stephen K. Robinson, Andrew S. W. Thomas, Wendy B. Lawrence, and Charles J. Camarda. Designated Station assembly flight LF1, the mission delivered supplies and equipment to the ISS aboard the Italian-built Multi-Purpose Logistics Module Raffaello.
Foam shedding and a second grounding
Cameras added after Columbia revealed that a large piece of foam — roughly 0.9 pounds (about 0.4 kg) and measuring some 36 by 11 by 7 inches — broke away from the external tank about 127 seconds after liftoff. The debris did not strike the orbiter, and Discovery was undamaged, but the event demonstrated that the redesigned tank had not eliminated the hazard that had doomed Columbia. On July 27, one day after the launch, NASA announced that it was again halting all shuttle flights until the foam-shedding problem could be resolved. The fleet did not fly again until STS-121 in July 2006.
On-orbit inspection and in-flight repair
The mission pioneered new safety procedures. As Discovery approached the ISS on July 28, Commander Collins guided the orbiter through the first Rendezvous Pitch Maneuver — a slow backflip that exposed the underside of the vehicle so that the station’s Expedition 11 crew, Sergei Krikalev and John Phillips, could photograph its heat shield for damage assessment. The crew also used a new sensor-equipped extension to the robotic arm to survey the wing leading edges.
During three spacewalks totaling roughly 20 hours, Noguchi and Robinson tested tile-repair methods and restored station hardware. On the third spacewalk, on August 3, imagery showed two thin “gap fillers” protruding from between thermal-protection tiles on the orbiter’s belly, raising concern that they could disturb airflow during re-entry. Anchored to the station’s robotic arm, Robinson maneuvered beneath Discovery and gently pulled the two strips free by hand — the first time repairs had ever been performed on the exterior of a crewed spacecraft in flight.
Return to Earth
After undocking on August 6, Discovery landed at Edwards Air Force Base in California at 8:11 a.m. EDT on August 9, 2005, having been waved off from its primary landing site at Kennedy Space Center because of unfavorable weather. The mission lasted 13 days, 21 hours, 32 minutes, and 48 seconds.
Significance
STS-114 was a milestone in NASA’s recovery from the Columbia disaster and a public demonstration that the agency could return astronauts to orbit while implementing the Columbia Accident Investigation Board’s recommendations. The flight validated new capabilities — in-flight thermal-protection inspection, the Rendezvous Pitch Maneuver, and the first hands-on repair of an orbiter’s exterior — that reshaped how the remaining shuttle missions managed risk.
At the same time, the recurrence of foam shedding and the resulting second grounding reinforced doubts about the shuttle’s long-term viability. The mission’s mixed outcome fed into the policy trajectory already set by President George W. Bush’s 2004 Vision for Space Exploration, which called for retiring the Space Shuttle after completion of the International Space Station. STS-114 thus marked both the resilience of the shuttle program and an early step toward its planned retirement, ultimately carried out in 2011.
